How to Sleep After Facelift Surgery

By Stephen E. Metzinger, MD, MSPH, FACS, triple board-certified plastic and facial plastic surgeon, Aesthetic Surgical Associates, Metairie, LA. Updated October 2026.

Quick answer: After a facelift, sleep on your back with your head and upper body raised about 30–45 degrees (two or three pillows, a wedge pillow, or a recliner). Keep this up for about 2 weeks, while swelling is at its worst. Avoid sleeping on your side or stomach until your surgeon clears you, typically 2–4 weeks after surgery, because pressure on the face and incisions can increase swelling and strain healing tissue.

Key takeaways

  • Back sleeping, head elevated, is the rule for the first 2 weeks.
  • Swelling and bruising are worst in the first 48 hours; elevation helps them drain.
  • Side sleeping usually returns at 2–4 weeks, once cleared.
  • Wear your compression garment at night as instructed.

What is the best sleeping position after a facelift?

On your back, with your head above your heart. Elevation lets fluid drain away from the face, reduces swelling and throbbing, and keeps pressure off the incisions in front of and behind the ears. Keeping your head straight, not turned or bent forward, avoids tension on the neck.

How long do I need to sleep elevated after a facelift?

Time after surgerySleep and recovery
Days 1–2Back only, head elevated 30–45°, compression garment; swelling and bruising peak
Week 1Continue elevation; short walks during the day
Week 2Swelling subsiding; garment continued; still on your back
Weeks 3–4Most symptoms gone; side sleeping often allowed once cleared
After week 4Normal sleep for most patients

How to set up your bed for facelift recovery

  • Wedge pillow or 2–3 firm pillows to keep your upper body raised.
  • Travel (U-shaped) pillow to stop your head rolling to the side.
  • Pillows under your arms or a body pillow on each side to keep you from turning over.
  • A recliner works well for the first nights if pillows are uncomfortable.
  • Silk or satin pillowcase to reduce friction on hair and skin near incisions.
  • Keep water, medication and phone within reach so you do not twist or bend.

Tips for sleeping better while you heal

  • Take prescribed pain medication before bed in the first days.
  • Avoid caffeine late in the day and alcohol while healing.
  • Use cold compresses during the day as instructed to calm swelling.
  • Take short walks; daytime movement helps nighttime sleep.
  • Do not smoke; nicotine slows healing of the facelift skin.

Does the type of facelift change how I sleep?

The sleep rules are the same, but recovery length can differ. Dr. Metzinger performs SMAS, deep plane and shorter “weekend” facelifts; your instructions are tailored to the technique used.

When should I call my surgeon?

Call right away if one side of the face suddenly swells much more than the other, if you have increasing pain, bleeding, fever, or spreading redness. Sudden one-sided swelling can be a hematoma, which needs prompt treatment.

Frequently asked questions

When can I sleep on my side after a facelift?

Usually 2–4 weeks after surgery, once your surgeon confirms you are healing well.

What happens if I accidentally sleep on my side?

One night on your side usually causes extra swelling, not damage. Return to back sleeping and mention it at your next visit; call if you notice sudden one-sided swelling or pain.

Can I sleep flat after a facelift?

Not in the first 2 weeks. Lying flat lets fluid pool in the face and increases swelling.

Do I sleep in my compression garment?

Yes, if it is part of your instructions; it supports healing tissue and limits swelling.
