The Wise-pattern breast reduction technique involves making an anchor-shaped incision – around the areola, vertically down to the breast crease, and horizontally along the crease. Breast tissue, skin, and fat are removed. The nipple-areola complex is lifted and repositioned. This technique allows for significant breast reshaping in addition to volume reduction. It is commonly used for major corrections.

This technique also involves an anchor incision. However, the nipple-areola complex remains attached to the inferior pedicle (a stripe of tissue in the lower breast), which provides the blood supply and ensures nipple viability. Unwanted tissue is removed from the top and sides of the breast. This technique preserves nipple sensation and the ability to breastfeed after surgery. It is often used for moderate reductions.

The surgery is performed under general anesthesia. This means the patient is fully asleep and pain-free during the procedure. The anesthesiologist provides anesthetic agents through an IV line and monitors all vital signs throughout the operation.
The surgeon makes a Wise-pattern incision to create the new breast contours. The length and pattern of the cuts depend on the amount of tissue that needs to be removed. This incision is carefully measured and marked before it is done. An inferior pedicle (a stripe of tissue in the lower breast) is preserved to maintain blood supply to the nipples and areolae.
Next, the surgeon removes excess glandular tissue, fat, and skin. The remaining breast tissue is reshaped and lifted to create smaller, lighter breasts that are proportional to the body.
The nipples and areola are left attached to the underlying breast tissue through an inferior pedicle. It contains the blood vessels that preserve sensation and milk duct function. The nipples and areolae are then repositioned higher on the breast mound and resized.
Once the breasts have been reduced and reshaped, the surgeon closes the incisions with sutures. Temporary drainage tubes may also be placed to prevent fluid buildup. The incisions are dressed with bandages that support the newly shaped breasts as they heal.
