He often uses Endotine® – a small, biocompatible piece of plastic that holds the brow in the proper place and gradually dissolves over the next six months as the tissue heals. The effects of a minimally invasive endoscopic brow lift can last for 7-10 years or more.
This traditional brow lift involves a long incision from ear to ear, typically hidden within the hairline. It provides a more extensive correction to the entire forehead and brow area, addressing both sagging and deep wrinkles. The procedure offers long-lasting results but may require longer recovery than others.

Preparation for a Brow Lift

A Step-by-Step Guide

Anesthesia
The surgery begins with either general anesthesia or local anesthesia with intravenous sedation. The choice depends on patient preference, Dr. Metzinger’s recommendation, and medical considerations. Our anesthesiologist monitors vital signs throughout the procedure to ensure safety and comfort.
Dr. Metzinger makes 3-5 small incisions (approximately 2 cm each) behind the hairline. These cuts are strategically placed: two in the temporal region on each side and one or three along the midline of the scalp. They allow Dr. Metzinger to insert an endoscope (a thin tube with a camera) and surgical instruments while minimizing visible scarring.
The endoscope provides a clear, magnified view of the underlying tissues on a video monitor. This technology allows for precise operations while maintaining minimal invasiveness.
Dr. Metzinger uses specialized instruments to release the periosteum (the tissue covering the bone). This process creates a dissection plane extending from the hairline to the orbital rim (bony edge of the eye socket) and across the forehead.
The muscles that cause frown lines are typically weakened or partially removed to reduce their ability to create furrows between the brows. This step helps achieve a more relaxed appearance and longer-lasting results.
The forehead tissues are lifted to their new position. Endotine® (a bioabsorbable fixation system) is inserted through one of the incisions and anchored to the skull. Its multiple points of attachment secure the elevated tissues in their new position until natural healing occurs. Multiple Endotine® devices may be used depending on the specific needs of the procedure.
The forehead skin and deeper tissues are carefully redraped over the Endotine® system. Dr. Metzinger ensures proper positioning and symmetry of the brows while maintaining a natural-looking result. Any excess skin tension is distributed evenly across the forehead.
The incisions are meticulously closed using sutures or surgical staples. Care is taken to minimize tension on the closure to promote optimal healing and minimize scarring. Hairline incisions are typically placed to ensure any resulting scars are well-concealed within the hair.

Recovery After
Brow Lift Surgery

First 24 Hours
Patients experience swelling and bruising around the eyes, forehead, and temples. Pain medication helps manage discomfort. The head should be kept elevated, and cold compresses can reduce swelling. Bandages remain in place, and patients should rest with minimal movement. Some numbness and tightness in the forehead area are expected.
Stitches or staples are typically removed within 7-10 days. Bruising begins to subside, though swelling continues. Patients usually return to light activities and work after 7-10 days. Mild discomfort and numbness persist. Following Dr. Metzinger’s instructions, hair can be gently washed after the first few days. The sleeping position must remain elevated.
Most swelling resolves, though some residual symptoms may persist. Normal sensation gradually returns to the forehead and scalp. Patients can resume most regular activities, including moderate exercise. Incision sites continue to heal and fade. Sun protection is crucial to prevent scarring.
Complete healing takes several months. The final brow lift results become visible as all swelling resolves. Incision lines continue to mature and fade for up to one year. Depending on the aging process and lifestyle factors, results may last nearly 10 years.
